Web Site Design & Service Provider Agreement

This document specifies the agreement between WestonMatheny.com (“Weston Matheny”) and _________________ (“_________________”).

Weston Matheny agrees to:

 Provide web design, hosting services, and up to six (6) email accounts for ____________ for a set monthly fee of $399.00, including:

1. Initial site design chosen from provided templates
2. Rotating flash banner, video and current ad on Home page
3. Up to 126 product images selected by _______ with category listings and logo gallery
4. Page to capture visitor’s name, email address and phone number
5. Finance offer and link to your finance company
6. Ability to feature pictures of your staff, store or showroom
7. Google Maps for driving directions to your store
8. Analytical reporting
9. Social Follow button to send visitors to your social networking sites

 Provide the above services and price for a period of at least 12 months

 Provide ________________ with at least 90-days written notice should Weston      Matheny opt out of this agreement

________________ agrees to:

 Utilize Weston Matheny as their web site design and service provider

 Provide a credit card for set up fees and recurring monthly billing of the monthly fee of $399.00 per month, plus additional options ________________ has chosen in the amount of $_________

 Commit to the above services for a period of at least 12 months

 Provide Weston Matheny with at least 90-days written notice should ______________opt out of this agreement
